Service with over twenty-five years of management experience;
your dedicated Customer Service Representative with over
ten years in insurance claims customer service; COBRA and
Retiree Services Manager also with more than ten years of
insurance administration and your Billing/Accounting Services
Department with many years of insurance billing experience
to assist you with your administration and customer service
requirements. In today’s medical climate of streamlining
and reducing staff for greater cost controls, quality service
can sometimes suffer. Some organizations feel a need

on-line management system with the following capabilities:

Your HR Department, through their own secure log-in, will
be able to view and make changes to your employees’ eligibility,
review benefits reports and interact directly through the
website with your Customer Service Manager.
Eligibility Administration
Our benefit administration services include:
- Day to day interface with the employer/employee regarding
eligibility, billing premium and claim issues.
- Enrollment Services – Web enrollment through our
FA on-line eligibility system
- Eligibility Status Changes – Compliance review,
tracking, processing and carrier notification
- Forms generation and processing – Enrollment
forms, confirmation statements, and dependent verifications.
- Monthly Bill Processing – Multiple carrier single
billing system, premium statement and eligibility audits.
COBRA Administration
- Produce and send initial notification letters to all
employees and dependants who become eligible after Fiabane
Associates begins administration.
- Produce and send Qualifying Event Notifications, which
include general information about COBRA, and enrollment
form, a premium calculation form and a HIPAA certificate.
- Accept and process payments from the qualified beneficiary.
- Work directly with your insurance carriers to report
eligibility, changes and terminations.
- Generate detailed monthly reports for clients and insurance
carriers.

the Plan participants is an important responsibility. We
collect non public personal information about Plan participants
from the following sources:
- Information we receive from participants on applications
or other forms.
- Information gathered from Client Services Calls are utilized
in analysis of data gathered regarding issues that may
help us develop better programs and reporting to enhance
our products and services.
- Information about participants’ transactions with
us, our affiliates or others.
We do not disclose any nonpublic personal information about
Plan participants or former Plan participants to anyone,
except as permitted by Law.
Confidentiality and Security
We restrict access to nonpublic personal information about
Plan participants to those employees and those affiliated and
non-affiliated third parties who need to know that information
to provide services participants. We maintain physical,
electronic and procedural safeguards that comply with Federal
regulations to guard their nonpublic personal information. |
